It happened to me once at CP. I was dealt a flush in 5-play and held all 5 cards. One card came unstuck but I surprisingly made the flush on 2 of the 5 hands. It took forever to get an attendant but he opened up the machine was able to confirm immediately that I had indeed been dealt a flush. He called a supervisor who insisted that the slot tech run some tests to see if the buttons could come unstuck. The slot tech did a number of things with the buttons and they both concluded that the buttons were faulty. The supervisor paid me the 90 credits I had lost and then shut the machine down.
